Community Health Representatives
Home visits, blood pressure and diabetes checks, medication reminders, and rides to appointments for those who cannot get in on their own.
CHRs are the part of the health system that comes to you. They check in on elders and on people managing a chronic condition, take blood pressure and blood sugar readings at home, and make sure prescriptions are being filled.
